Job Description Summary:

The Registered Nurse within our Growth and Development Department serves as a liaison for Solaris, bridging the gap between patient care provision and community outreach across our hospice, home health, and palliative care departments. This position embodies our commitment to healthcare delivery by engaging with healthcare professionals, physicians, community groups, and individuals/families, while also providing compassionate nursing care to those in need.

Essential Job Functions/Responsibilities:

  • Conducts initial and comprehensive assessments of patients and their families to determine healthcare needs, encompassing hospice, home health, and palliative care services.
  • Provides professional nursing care utilizing the nursing process, ensuring a holistic approach that addresses physical, emotional, and spiritual needs.
  • Develops and revises individualized care plans based on ongoing assessments, nursing diagnoses, and palliative nursing actions, actively involving patients and families in the planning process.
  • Administers medications and treatments as prescribed by physicians and provides health education and counseling to patients and their families.
  • Engages with healthcare professionals, physicians, and community groups to establish referral relationships and educate them about the services offered by our hospice, home health, and palliative care departments.
  • Develops and conducts in-service and continuing education events at various healthcare facilities, physician offices, hospitals, home health agencies, and throughout the community to promote awareness and understanding of our services.
  • Actively participates in interdisciplinary team meetings, care conferences, and healthcare provider committees to promote collaboration and coordination of care across departments.
  • Maintains up-to-date knowledge of rules and regulations pertaining to hospice, home health, and palliative care, and participates in orientation and ongoing education programs.
  • Manages provider relations, fostering open communication and collaboration with healthcare professionals and organizations.
  • Adheres to Standards of Conduct.
